    Full-Time Offer Report Form

    Once you have received an offer for full-time employment after graduation, please fill out the following form. This data is used for the purposes of creating full-time salary averages, an important tool used in Rose-Hulman recruiting efforts. Your specific salary information will not be shared individually. NOTE: If you submit a form for an open offer, please contact Career Services once you have made a decision about it.

    Signed by your Advisor, this is your official application into our co-op program. Must be completed before you leave for co-op the first time.

    Signed by your Advisor, this is your plan for fitting co-op work into your program of study. Must be completed before you leave for co-op the first time.

    Notice of Acceptance & Departure Form
    Signed by various on-campus offices, this notifies them that you will be leaving campus to work, but will remain a full-time student. This form also provides us with important contact information for you while you are on co-op. Must be completed for each non-continuous co-op assignment. If your assignment or information changes while you are on site, please submit a new form.

    Student Mid-Term Evaluation
    Filled out at the mid-point of your work term, this provide us with information about your experience.

    Student Evaluation
    Filled out at the conclusion of your work term, this provide us with information about your experience. It is filled out at the end of each work term that is not continuous. Must be completed within 4 weeks of the end of your co-op term.

    Student Co-op Reflection
    Filled out at the conclusion of your work term, this gives you an opportunity to see the areas you gained experience in, and gives us an idea of how other students might also benefit from a similar co-op experience. It is filled out at the end of each work term that is not continuous. Must be completed within 4 weeks of the end of your co-op term.

    Employer Evaluation
    At least 4 weeks before the end of your work term you should ask your supervisor to complete this evaluation. Their evaluation will provide valuable feedback about your performance and should indicate areas for improvement in future co-ops or jobs. It is filled out at the end of each work term that is not continuous. Must be completed within 4 weeks of the end of your co-op term. Can be submitted by the supervisor or the student.

    Q & A about Forms

    What does non-continuous mean for my work terms?
    If you go on co-op for 2 or more quarters in a row, that is a continuous work term. If you go on co-op for one term, then return to school for a term, and then go out on co-op again for another term, that is non-continuous.

    Where do I send the forms?
    You can send them by regular mail, email or fax.

    Whose responsibility is it to get these forms done?
    As the student you are responsible for either downloading the forms from this page, or contacting the Co-op Coordinator to have them sent to you. Following up on forms, including the employer evaluation, will be expected for you to have a complete co-op experience.
