Accident/Illness Health Insurance

Student Health Insurance- Accident and Injury Insurance is automatically charged to all students unless proof of insurance is provided to our Risk Management Coordinator. In order to waive the insurance charge the student/parent must complete a waiver form at the start of the school year and submit this form to our Risk Management Coordinator before the tuition payment deadline. Insurance charges cainnot be pro-rated or removed from the Student Billing invoice after the tuition payment deadline. This policy is an excess policy that will pay eligible expenses after any other collectable insurance has been exhausted. The bills incurred by the student must be submitted to the family's insurance policy first.

Domestic Insurance Claim Procedures

A claim form must be completed and submitted to our Risk Management Coordinator before the claim may be submitted to our insurance carrier. Claims will not be paid unless an institute claim form is completed. Our carrier will not accept ai universal claim form from a provider. International Insurance Claim Procedures

- A claim form must be completed and submitted to our Risk Management Coordinator before the claim may be submitted to our insurance carrier. In order to decrease the deductible charge, an insurance referral letter should be obtained from our campus physician. Please review the Deductible section in the Rose-Hulman International Student brochure for an explanation of deductibles, exclusions, medical benefits and claim procedures.

Athletic Insurance

Athletic Insurance is available for every student that participates in an Intercollegiate, club or Intramural sports. These policies are excess insurance coverage that will pay eligible expenses after any other collectable insurance has been exhausted. The bills incurred by the student athlete must be submitted to the family's insurance policy first. The Rose-Hulman Student insurance plan will pay towards intramural and club sport injuries but it will not cover intercollegiate injuries.

Intercollegiate Sport Insurance

All claims made through the intercollegiate policy must be filed by a Rose-Hulman Athletic Trainer. signed by the student and submitted to our Risk Management coordinator. This policy has a $500.00 deductible which must be paid by the family or the family insurance program. Please review the intercollegiate Athletic Insurance brochure for an explanation of coverage, deductibles, exclusions and claim procedures.

Intramural and Club Sports Insurance

This is a catastrophic insurance policy with a $25,000 deductible. This policy offers insurance coverage when a student is participating in a club or intramural covered sporting event. This policy does not offer coverage for injuries sustained during a club or intramural practice session. All claims made through this policy must be completed by a Rose-Hulman Athletic Trainer, signed by the student, and then filed with the Risk Management Coordinator. The student will also need to complete a Data Fact Sheet and return this form to the Risk Management Coordinator before the claim can be filed with our insurance carrier. Property and Liability Insurance

Automobile Policy

RHIT owned and non-owned leased vehicles are insured bv Rose-Hulman Institute of Technology and only authorized licensed individuals may drive these vehicles. To qualify to drive a RHIT owned. leased or rental vehicle. the individual must furnish our Risk Management Coordinator with a copy of their drivers license. A Motor Vehicle Report (MVR) will then be obtained to verify past driving history. If an MVR comes back that is considered to be a risk. the employee will be restricted from driving institute owned vehicles, leased or rental vehicles until further notice.

If an employee or student should have an accident while in an institute owned, leased or rental vehicle, the person should contact the local authorities and our Risk Management Coordinator (xS457) immediately. If the accident happens on campus. the employee/student should also contact our Campus Security (x891 1) so an incident report may be completed.
Steps to follow in an accident:
  1. Stop immediately, do not leave the scene of the accident and do not move your vehicle until told so by the police.
  2. When the accident occurs off campus, call the local police in the accident area and Risk Management (x8457).
  3. For accidents occurring on campus, contact Campus Security (xS9lI) and Risk Management (x8457).
  4. Exchange identification and insurance information with the other driver(s).
  5. DO NOT give statements to anyone except the police, our RHIT Risk Management Coordinator and RHIT Campus Security. Statements may be given to our insurance carrier and any other insurance carrier involved in the claim after a report has been filed with Risk Management (xS457).
  6. Be courteous, cooperative and reasonable
  7. Do not admit nor deny to blame. Do not sign anything.
  8. Contact Risk Management to notify the University and file a claim for damages.
  9. Contact Campus Security to file an on-campus accident claim.


Personal Injury -Persons involved in an automobile accident should seek medical help immediately if necessary.
Issues to consider:
  1. If injured in an auto accident while on Institute business in the Terre Haute area/vicinity and you need medical care, go to Occupational Health on East Wabash or Union Hospital emergency.
  2. If you are out of town and need medical care, go to the nearest health care provider or Emergency room.
  3. Personal injury of students and visitors on Institute property must be reported to Campus Security (x891 1) and Risk Management (x8457).


Certificate of Insurance

A certificate of insurance is required for companies, vendors, independent contractors and individuals that provide a service or who use or lease Institute owned space. This also applies to companies, vendors or contractors that provide performance testing of equipment, such as sprinkler system tests and water samples.

Rose-Hulman Institute of Technology must be listed as the certificate holder and as an additional insured.

When an individual, corporation, vendor or independent contractor use or lease space from Rose-Hulman, a Hold/Harmless agreement must be completed and returned to our Risk Management Coordinator. The completed Hold/Harmless agreement and certificate of insurance must be received by our Risk Management Coordinator 14 days prior to the date of the event. Clients are not allowed to use our facilities without this information on file.

General Liability

The Rose-Hulman Institute of Technology general liability policy provides coverage they are lega1y obligated to pay (subject to exclusions) for liability arising out of premises/operations, product liability, completed operations and personal injury.

Assumption of Risk and Release

Rose-Hulman Institute of Technology has an Assumption of Risk and Release Waiver form to be completed by students, faculty, staff and visitors when they participate in a Institute event off institute-owned property. The institute has a "duty-to-warn" individuals of the potential risks they may encounter \\'hile participating in an off-campus event.

Students, alumni, employees and visitors who participate shall sign the Assumption of Risk and Release Waiver form. This form should then be sent to our Risk Management Coordinator.

Travel Abroad - Faculty and staff members who are required by their job description to travel abroad will have liability coverage under a Rose-Hulman liability policy. Sabbatical abroad does not qualify as a job requirement.

Rose-Hulman does not have liability coverage to cover students when they travel abroad. Insurance coverage for traveling abroad is the responsibility of the student and can be purchased from any insurance agent or broker. Our RHIT Accident/Illness insurance plan does cover medical fees while traveling abroad and also includes medical evacuation and repatriation of remains coverage.

D & 0 Insurance

Liability insurance protects Institute employees if they are named in a lawsuit for an alleged error or omission they made when performing their duties for the Institute. There is no coverage for a criminal act.

Property Coverage

Our property coverage has a $5,000 deductible per claim.

Electronic Data Processing Floater - This is an additional policy that offers a $1,000 deductible per claim for computer and phone equipment when the insured value per items is valued over $500.00 after depreciation. Items are automatically added to this policy based on account codes used on the purchase order at the time of the order. This policy tracks non-capital and capital expenditures coded to 7840.7845, 7921, 7925, 7940 and 7945 commodity codes.

Art Floater - This policy covers our personal art collections and items on loan to us by artists. Items on loan or added to our collection must be reported to our Risk Management Coordinator (x8457). The Risk Management Coordinator will need to know the description of the item(s) including size and title, appraised value, artist, location and if the item is owned by the institute or on loan. If the item is on loan, you must provide the dates that insurance coverage is required. If Risk Management is not notified of the item(s) to insure, then there is no coverage.

Contractor's Equipment Floater This is extra insurance coverage to cover equipment and tools that are used 1,000 feet from the primary storage location. This would cover landscaping equipment, motorized carts, weed eaters and etc. In order to have coverage under this policy, the item must be specifically listed in the policy.

Worker's Compensation Insurance

General Information

Worker's Compensation is a law mandated by the State of Indiana to provide insurance coverage for all employees who may be injured on-the job. The purpose is to help the work-injured employee recover to pre-injury status and return to the workplace.

Claim Procedures - All employees or student workers who are injured during working hours are required to report the injury to their immediate supervisor immediately. A First Report must be filed and faxed to our Risk Management Coordinator (fax #8032) within 24 h ours of the date of injury. The injured employee/student worker are required to obtain medical service from Occupational Health. If Occupational Health is closed, you must seek medical attention from Health Check East. If both of these locations are closed or if the injury is life threatening, then report directly to Union Hospital emergency room. Your immediate supervisor at the time of notification of the injury will furnish you with an insurance packet that at you must take with you to the designated health care provider. You must only seek medical attention from our designated provider.

Failure to follow the procedure by seeking your own medical care will result in non-payment of medical bills and you will be responsible for the incurred medical costs. If your injury requires a medical subscription, do not use your Rose-Hulman prescription card. You may either, pay for the prescription and turn your receipt in to our Risk Management Coordinator for reimbursement, or contact the Risk Management Coordinator who will put you in contact with our insurance adjuster. The insurance adjuster will then make arrangements with a local pharmacy to have your prescription filled. Compensation - You must be off from work seven (7) days before worker's compensation will compensate you for your injury. After you are off twenty-one (21) days, worker's compensation insurance will pay you for the first seven (7) day waiting period. Your days off from work must be at the request of the designated facility/doctor in order to receive any compensation. Worker's compensation payment is 66 2/3 % of your average wage based on the last 52 weeks worked not to exceed an amount specified by the State Workers' Compensation law, until the employee returns to work.
